Individualized Home Supports with Training

Home and community-based IHST services help people achieve their goals and maintain their independence. We are committed to holistic and healthy choices. We advocate for those we serve to navigate government programs, health care teams, and other professional and social support groups.

Primary Duties and Responsibilities:
  • Provide information, coaching, and support to assist clients in achieving their goals
  • Accompany clients to medical appointments, meetings, hearings, reassessments, etc.
  • Assist clients with documentation related to federal, state, and county benefits, including Medical Assistance, Social Security, HRA housing recertifications, Metro Mobility Applications, Energy Assistance applications, etc.
  • Assist clients in identifying resources within the community that fulfill their particular needs
  • Participate in all required BeechWood staff and program meetings, applicable training activities, and client events
  • Provide accurate and timely mandatory reporting (e.g., client narratives, client progress reports, employee timesheets, etc.)

Qualifications:
  • Experience with a disability and/or knowledge of disability issues and the client/community
  • Ideal candidate should be self-motivated, comfortable working independently, have strong interpersonal skills, and the ability to communicate effectively and professionally
  • Basic computer skills
  • Minimum of one year working in the Human Services field required; BA/BS degree in Human Services or related field preferred

Additional Requirements:
  • Reliable transportation, valid driver's license with a clear driving record, and proof of insurance
  • Must complete and pass a criminal background examination
  • The ability to lift weight in excess of 25 pounds
